OUR IDEAL CANDIDATECenter for People With Disabilities (CPWD) is looking for an Independent Living Advisor. This is a dynamic and rewarding role where you get to work with people with different types of disabilities, different life circumstances, and different independent living needs. Who we’re hoping to find is someone living with a disability who is also familiar with Independent Living Philosophy. Don’t worry, we can teach you, but you need to be passionate about assisting people with disabilities at achieving independent living. This position works best with someone who has a personable way of relating to people where they are at – everyone comes with a story, an obstacle, and a need. Having the ability to listen and understand, and then assist individuals at setting goals is key. This is not a routine, fill-out-a-form, position; you must love working with people for who they are. Because the work is dynamic and person-centered, you’ll also need time-management and activity tracking skills so you can keep track of consumers, goals, your time, follow-ups, and forward planning. And finally, we are looking for someone who likes to have fun with others – we want you to come up with creative ideas for peer groups, outings, and other social time that will be fun, educational and inspiring. SUMMARY: The Independent Living Advisor (ILA) works directly with people living with disabilities within our six county service region. Responsibilities include speaking with current and potential participating consumers to gather information and to create an Independent Living Plan by identifying areas of need. Based on this plan, the ILA will then help the consumer identify necessary resources through assisting in the completion of paperwork, applying for benefits, and pursuing avenues of self-advocacy. Duties may also include teaching Independent Living skills and providing peer support, in one-on-one and small group settings. The individual in this position will serve as a role model and mentor for people with disabilities within the community, and maintain positive and effective relationships with CPWD staff and partner agencies.

JOB TITLE: Administrative Assistant
REPORTS TO: Director of Operations
PRIMARY OFFICE: Boulder
FSLA/EMPLOYEE STATUS: Full-Time (32 hrs. /wk.; 4 days/wk.); Non-exempt

SUMMARY: Responsible for handling accounting functions for payroll and month-end accounting activities, such as reconciliation of credit cards and bank accounts. Assists with administrative operations for all of CPWD’s offices. Provides administrative assistance to the Executive Director. The individual in this position will serve as a role model and mentor for people with disabilities within the community, and maintain positive and effective relationships with CPWD staff and partner agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Collect, enter, and process all staff timesheets.
· Process and submit biweekly payroll.
· Track and report employee benefits.
· Perform monthly reconciliations of bank, credit card, and petty cash accounts.
· Maintain all accounting and employee files in accordance with internal policies and government regulations.
· Assist with periodic audits.
· Assist Executive Director with calendar management, travel planning, conference registration, meeting management, and other specific activities, as assigned.
· Assist all departments by coordinating administrative tasks, documentation, and communication
· Assist with coordination relating to: service providers (e.g., document shredding); building cleaning, maintenance, inspections, and repairs; and office equipment, including phones, copiers, computers, and network equipment.
· Order and/or pick up any food or supplies needed for the office or programs, as needed.
· Assist with set up and clean up for meetings and events, including tables, chairs, food, and refreshments, etc.
· Assist with all aspects of the office’s space/infrastructure planning (moves, adds, and changes to workstations), and provide answers, resources and solutions, as requested.
· Order postage, office supplies, cleaning supplies, and other miscellaneous supplies.
· Help ensure office cleanliness and safety.
· Maintain strict confidentiality in all areas, work within the confines of HIPAA regulations, and complete all required HIPAA trainings.
· All CPWD staff members are considered mandatory reporters, obligated to report signs of abuse they are aware of both on and off the job.
· Work independently, schedule and manage personal workload, and recognize when there is a need for approval or support from supervisor.
· Gain an understanding of IL Philosophy and CPWD’s mission, vision and programs to be able to communicate them to consumers and community organizations.
· Participate in CPWD staff meetings, department meetings, and staff trainings.
· Perform other duties, as assigned.
REQUIRED QUALIFICATIONS:
· 2+ years of experience in office management, administrative assistance, and payroll.
· Should have good organizational skills with a strong attention to detail.
· Must have excellent oral and written communication skills and ability to interact with associates in remote locations through email, phone, or video.
· An ability to interact well in a team environment is essential.
· Must be able to demonstrate dependable and reliable attendance and performance, work flexibly and creativity with ability to multi-task.
· Computer literacy with proficiency in MS Word, Excel, Google Suite, and web-based portal systems is essential.
· Leadership and interpersonal skills with diverse peers, consumer base, other staff members and management.

TITLE: Beyond Vision Skills Trainer

SUMMARY: Implements the Beyond Vision program for people with visual impairments or blindness by facilitating low-vision support groups, providing one-on-one skills training, and meeting outreach goals. The individual in this position will serve as a role model and mentor for people with disabilities within the community, and maintain positive and effective relationships with CPWD staff and partner agencies. Primarily provides Beyond Vision services to the Denver Metro region but may also need to serve the Boulder County region, as neededESSENTIAL DUTIES AND RESPONSIBILITIES:
• Coordinate and facilitate regular peer support groups and workshops for community members. Encourage peer support and social networking within each group, and teach advocacy skills when appropriate.
• Assist consumers in learning adaptive techniques, coping strategies, and eye health to promote their independence in their home and community.
• Provide individual services to consumers with low or no vision including marking appliances, independent living skills training, basic travel skills, basic assistive technology, and assistance with completing grant applications for low-vision equipment funding.
• Participate as a team member with the entire Beyond Vision team.
• Engage new consumers and community partners through outreach. This may include hosting small outreach events at specific locations, attending community fairs, and working directly with staff at senior living communities and recreation centers.
• Work collaboratively with other CPWD programs and human service agencies to refer consumers to appropriate providers.
• Maintain accurate and timely paper and electronic records required by CPWD, including Consumer Services Records (CSR) files and grant paperwork.
• Maintain strict confidentiality and work within the confines of HIPAA regulations.

JOB TITLE: Transition Coordinator

SUMMARY: The Transition Coordinator is responsible for assisting individuals with disabilities to transition out of long-term care facilities into living in the community with supports and services. The Transition Coordinator will be responsible for guiding consumers seamlessly through the transition process, providing support throughout and conducting follow-ups afterward to ensure optimal access to services. The scope of transition services includes assessing not only medical/health needs but also the consumer’s community living and social needs such as assisting with finding housing; setting up non-medical transportation; helping the individual integrate into the community through clubs, volunteering/work, faith organizations, etc. Will serve as a role model and mentor for people with disabilities within the community, and maintain positive and effective relationships with CPWD staff and partner agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES:· Provide one-on-one direct services to consumers, including the core services of Independent Living Skills Training, peer support, advocacy, and information & and referral services.
· Act as an advocate for and with consumers.
· Facilitate Transitions team meetings between the consumer, nursing facility staff, and other community partners.
· Complete a transition assessment, transition plan and risk mitigation plan with the transition options team and continue to review during the transition.
· Actively collaborate and communicate with physicians, social workers and service providers regarding discharge planning, appropriate follow up, and post-transition services for consumers.
· Coordinate community living services, housing, durable medical equipment, and purchase household items for those transitioning out of nursing homes through the Transitions program.
· Complete all state-mandated Transition Coordinator trainings and attend ongoing trainings.
· Participate in community outreach, including public speaking to groups, to promote the Nursing Home Transitions program and other CPWD programs.
· Work independently, schedule and manage personal workload, and recognize when there is a need for approval or support from supervisor.
· Maintain accurate and timely records required by CPWD, including Consumer Service Records, Medicaid databases documentation, and documentation of billable hours for services rendered.
· Must comply with policies, procedures and regulations from CPWD and funding sources related to use of company credit card, purchasing, purchase requests, and promptly submitting receipts.
· Must complete all Medicaid Transition Coordination trainings within agreed timeline and keep up with required annual trainings.
· Maintain strict confidentiality and work within the confines of HIPAA regulations.
· All CPWD staff members are considered mandatory reporters, obligated to report signs of abuse they are aware of both on and off the job.
· Must be able to travel as needed for direct services, groups, meetings, trainings, and outreach events.
· Participate in CPWD staff meetings, program meetings, and staff trainings.
· Perform other duties, as assigned.
REQUIRED QUALIFICATIONS:· Associate’s degree in a relevant field or relevant equivalent experience.
· Proficiency with MS Windows, MS Office, Google Suite, and other common computer programs.
· Must maintain current COVID vaccine status and practice precautionary measures as defined by regulatory authorities governing the program and provide vaccine documentation to HR.
· Strong organizational and customer service skills, and attention to detail.
