STEP Program

Support Toward Empowered Post‑trauma Independent Living

The STEP program is a pilot partnership between the Center for People With Disabilities (CPWD) and Boulder Community Health (BCH), designed to support people with disabilities who are transitioning from trauma care back into the community.

Many people leave the hospital after a traumatic injury without a clear pathway to the services that support independent living. Without that connection, navigating benefits, securing housing, and managing daily life can feel overwhelming.

STEP was created to bridge this gap by embedding independent living support into the recovery process so that individuals have the tools, knowledge, and community connections they need before and after discharge.

Who this program serves

STEP supports individuals recovering from traumatic injury or a significant health event who are preparing to return home, especially those adjusting to new or newly intensified disabilities.

How the program works

Support is tailored to each person's needs through a flexible tiered model:

Information & Referral

All eligible patients receive information about independent living resources, community programs, and services that may support their recovery.

Tier 1


Options Counseling & Resource Navigation

Tier 2

Individuals who want more hands-on support work with CPWD staff to explore benefits, housing, transportation, peer support, and skill-building opportunities.


Independent Living Services

Tier 3

Individuals with independent living goals become CPWD consumers and work directly with an Independent Living Advisor to set and achieve those goals.

How to join the program

STEP is currently available through referral only. If you or someone you know is being treated at Boulder Community Health following a traumatic injury, here's how the process works:

1. Referral from the BCH Trauma team

A member of the BCH Trauma unit identifies eligible patients and connects them with CPWD.

2. Introduction to an Independent Living Advisor

A CPWD advisor reaches out to share information about available resources and answer questions — no commitment required.

3. Choose your level of support

From a one-time conversation to an ongoing independent living plan, participants decide what kind of support is right for them.

STEP is not currently open to self-referral. If you have questions about eligibility, contact CPWD directly.
